Complete our New Patient Form to start your file. You’ll receive an email shortly after to set up your FMI Patient Portal. *Important: Each family member seeking care must submit a separate form using the patient’s name to ensure individual portal creation.

Complete Forms & Verify Insurance

Log into your FMI Patient Portal to upload your health history/intake forms and a clear photo of your insurance card (front and back). Telemedicine is available for new patients in: AZ, FL, ID, MT, OR, UT, WA, and WY.

Once your forms and insurance are verified, an FMI Care Coordinator will call you to schedule your initial consultation. This ensures all details are confirmed before your first appointment.

Frequently Asked Questions

Yes, for our Primary Care and Functional Care options, we bill office visits seamlessly to insurance. Our Specialty Care is offered on a cash-only basis.

For primary and functional care, our team of providers is in network with most Blue Cross, Regence, Aetna, Select Health, Pacific Source, Cigna, and Mountain Health Co-op plans.

Every insurance plan is different. To help you determine what your plan will cover at our office, we have put together a list of common appointment, lab and diagnostic codes used for billing purposes. We recommend calling your insurance and asking about the coverage of these commonly used codes: If you are needing CPT codes, please don’t hesitate to contact our office at (208) 385-7711 and select the Billing option when calling, or you can also reach us via email at billing@funmedidaho.com. We are more than happy to assist you with any inquiries related to CPT codes.

For insurances that FMI is not contracted with, you may request a superbill to submit to your insurance for reimbursement you as you will be a CASH pay patient. Your insurance reimbursement may vary, so please contact your insurance to verify that you hold out of network benefits. We accept HSA and FSA cards as a form of payment.
Primary Care focuses on acute illness, preventative screenings, and annual exams with a functional mindset. Functional Care is a deep-dive, problem-oriented approach for chronic issues that includes extended visits, advanced testing, and a comprehensive personalized plan, which involves an additional annual fee.
The $695 annual fee covers the extra time, enhanced access, and specialized services unique to our Functional Care model, such as extended visit times, interpretation of complex labs, and convenient messaging access to your medical team. This ensures we can dedicate the comprehensive time needed to manage chronic, complex conditions.
We accept Flexible Spending Accounts (FSA) or Health Savings Accounts (HSA) cards as a form of payment. We recommend checking with your plan administrator for coverage details.
Yes, many patients choose Primary Care as their home base while utilizing Specialty Care for specific treatments like hormone optimization or IV therapy. Your New Patient Coordinator can help you design the best plan.
