To start the process of becoming a patient with Functional Medicine Institute, please complete the inquiry form. After the submission of your inquiry, a link will be emailed to you to create your FMI patient portal.
If you are interested in care for multiple members of your family, be sure to fill out an inquiry form for EACH person. Each person will need their own FMI Patient Portal.
Functional Medicine Institute uses an online portal to provide patients with a convenient place to see all upcoming appointments and send/receive secure messages with your Provider and Care Team!
Telemedicine is available in the following states: Idaho, Oregon, Washington, Utah, Arizona and Florida.
Once you have registered with Functional Medicine Institute, please begin your New Patient Intake forms listed under the “Questionnaire” tab.
After intake forms are complete, please upload a copy of your insurance card (front and back) under the “Labs and Documents” tab. This page will allow you to easily take a photo of your insurance card with your phone, and upload.
Once you’ve submitted all intake forms in the patient portal and uploaded your insurance for verification, you will receive a Scheduling Call from an FMI Care Coordinator to schedule your initial consultation and begin your journey to healing!
During this Scheduling Call, a Care Coordinator will address any missing information in your patient portal, and can answer any questions you have.
Our team of providers is in network with most Blue Cross, Regence, Aetna, Select Health, Pacific Source, Cigna, and Mountain Health Co-op plans.
Out of Network Information: Most insurances not listed above are considered out-of-network. Out of network patients are considered self pay although FMI can provide you with a detailed bill for most services that contain ICD-10 diagnostic codes and CPT codes for reimbursement from your insurance. We accept all HSA and FSA cards as a form of payment.
Medicare & Medicaid:
Functional Medicine Institute is NOT contracted with Medicare and Medicaid.
Medicare and Medicaid patients are consider self-pay patients.
It’s important to note that Medicare and Medicaid will not reimburse for care at FMI.
