Yes, we are pleased to offer the convenience of accepting Health Savings Account (HSA) and Flexible Spending Account (FSA) cards as valid forms of payment. It is important to note, however, that the coverage of the FMI Membership by your HSA or FSA is subject to the policies and guidelines set forth by your respective healthcare financial account. We recommend you first verify the eligibility and coverage details with your HSA or FSA provider to ensure a seamless and informed payment process!

No. Discounts and perks are non-transferable to spouses, family members or friends. To have access to benefits and discounts, they must have their own individual FMI Membership.

We have a promo of 1 month free of Membership when you refer a friend or family member! Just have your friend/family member let the Care Coordinator know when they schedule their initial appointment for you to receive your free month.

Our team of providers is in network with most Blue Cross, Regence, Aetna, Select Health, Pacific Source, Cigna, and Mountain Health Co-op plans. 

Every insurance plan is different. To help you determine what your plan will cover at our office, we have put together a list of common appointment, lab and diagnostic codes used for billing purposes. We recommend calling your insurance and asking about the coverage of these commonly used codes: If you are needing CPT codes, please don’t hesitate to contact our office at (208) 385-7711 and select the Billing option when calling, or you can also reach us via email at [email protected]. We are more than happy to assist you with any inquiries related to CPT codes.

If your plan is in-network with Functional Medicine Institute, an FMI team member will be able to verify your eligibility and assist you with the following:

  • Copayment, coinsurance, and current deductible amounts
  • Estimated out-of-pocket charges per medical visit based on your plan
  • Reach out to [email protected] with questions.

Payment is required at the time of service for all co-pays, deductible retainer (if applicable), and any outstanding account balances. Deductible retainer is $150 retainer for all patients who have not yet reached their deductible. This retainer is applied to the total cost of the appointment. Any balance after claim is processed through insurance and retainer is applied, will be billed to the patient. Deductible retainer is not collected for most Wellness Appointments, but varies by insurance policies.

Labs are NOT included in the FMI Membership. This allows patients the freedom and choice to choose the lab they would like to be drawn at. FMI utilizes 3rd party labs for lab-draws. To find out which labs are in-network with your insurance plan, call the member service number on the back of your insurance card, or log in to your insurance provider’s member portal to find out which lab is preferred.

Many FMI Members find it helpful to ask their insurance representative the following questions:

  • I’m working with an out-of-network provider. Do I have out-of-network lab coverage?
  • Is there a preferred in-network lab provider?
  • What is my current deductible status?
  • What coverage can I expect for the testing my doctor wants me to get? (Ask your FMI medical team for a CPT document for your specific lab order, which will include both the billable CPT and ICD-10 diagnosis codes associated with your order. By providing these codes, a representative will be able to advise which tests are eligible for coverage based on your plan. Upon request, this CPT document will be uploaded to your FMI patient portal.

Functional Medicine Institute is NOT contracted with Medicare and Medicaid.

Medicare and Medicaid patients are considered self-pay patients. It’s important to note that Medicare and Medicaid will not reimburse for care at FMI.

For insurances that FMI is not contracted with, you may request a superbill to submit to your insurance for reimbursement you as you will be a CASH pay patient. Your insurance reimbursement may vary, so please contact your insurance to verify that you hold out of network benefits.

We accept HSA and FSA cards as a form of payment.

Month-to-Month: If you are paying monthly, you may cancel your plan after 3 months, before the next payment is processed. Payments are processed on the day of the month it was originally activated.

Paid in Full: If your FMI Membership was paid in full, no refunds will be issued but a cancellation fee will not be applied.
